AMONG OTHER ACTIVITIES, GLOW U MED SPA’S SERVICES ENABLE COORDINATION AND COMMUNICATION WITH LICENSED HEALTH CARE PROVIDERS. THESE SERVICES DO NOT REPLACE YOUR RELATIONSHIP WITH ANY PHYSICIAN. GLOW U MED SPA’S SERVICES MAY NOT BE APPROPRIATE FOR ALL MEDICAL CONDITIONS OR CONCERNS.

BY ACCESSING OR USING GLOW U MED SPA’S WEBSITES, PLATFORMS, OR SERVICES, YOU ACKNOWLEDGE THAT YOU HAVE READ, UNDERSTOOD, AND AGREE TO BE LEGALLY BOUND BY THESE TERMS OF USE. IF YOU DO NOT AGREE, YOU MAY NOT USE THE SERVICES.
INTRODUCTION
Glow U Med Spa, and any affiliated entities (“Glow U,” “we,” “us,” or “our”), own and operate our websites, online platforms, and any affiliated mobile applications (collectively, the “Sites”).
The Sites are intended to facilitate access to wellness, aesthetic, and telehealth-enabled services (the “Services”), which may include:
Health, wellness, and aesthetic educational content
Coordination of cosmetic, wellness, and medical services
Telehealth-enabled consultations with licensed providers
Scheduling, payments, and administrative support
Secure electronic communications between patients and providers
These Terms of Use govern your access to and use of the Sites and Services, along with our Privacy Policy and any additional consent forms you are required to accept.
AVAILABILITY & ELIGIBILITY
Certain Services may only be available in specific states.
To use the Services, you must:
Be 18 years of age or older
Be located in a state where the Services are offered
Agree to comply with these Terms of Use
Meeting eligibility requirements does not guarantee that you will receive services. Providers retain sole medical discretion.
YOUR RELATIONSHIP WITH GLOW U MED SPA
Glow U Med Spa does not itself provide medical care. Glow U provides a platform and administrative support that enables you to communicate with independent, licensed health care providers who deliver care.
Using the Services does not create a provider-patient relationship with Glow U Med Spa itself. Any provider-patient relationship exists solely between you and the licensed provider delivering care.
TELEHEALTH SERVICES & CONSENT
Telemedicine involves delivering health care services using electronic communications when the patient and provider are not in the same physical location. Telemedicine may include:
Transmission of medical information, images, and data
Secure messaging, audio, or video communications
Review of questionnaires and uploaded materials
Glow U uses reasonable technical and administrative safeguards to protect health information, but no system is 100% secure.
Additional risks, benefits, and limitations are explained below and form part of your informed consent.

By clicking “I Agree”, you acknowledge and consent to participating in asynchronous telemedicine services.
What Is Asynchronous Telemedicine?
Asynchronous telemedicine (also known as “store-and-forward”) allows you to submit information—such as photos, videos, messages, or questionnaires—that a licensed provider reviews at a later time.
This differs from live video or phone visits.
Potential Risks
Although uncommon, risks may include:
Delays due to technology failures or connectivity issues
Privacy or security breaches despite safeguards
Incomplete clinical information if you fail to disclose full medical history
Limitations due to lack of in-person examination
Asynchronous telemedicine is not emergency care.
If you experience a medical emergency, call 911 immediately.
Provider Discretion
All diagnoses, treatment decisions, and prescriptions (if any) are made solely at the provider’s discretion. There is no guarantee of treatment, diagnosis, or medication.
YOUR RIGHTS & ACKNOWLEDGEMENTS
You acknowledge that:
You have the same privacy rights as with in-person care
Telehealth may involve providers located in other states (as legally permitted)
All submitted information becomes part of your medical record
You may request access to your medical records by contacting Glow U Med Spa
You may withdraw consent at any time, subject to legal record-retention requirements
Health information may be disclosed as required by law, including:
Court orders or subpoenas
Mandatory reporting obligations
Prevention of serious threats to health or safety
TEXT, EMAIL & COMMUNICATION CONSENT
By providing your contact information, you consent to receive calls, emails, and text messages (SMS/MMS) related to appointments, services, treatment coordination, and administrative matters.
Messages may be sent using automated systems. Carrier rates may apply.
Consent is not a condition of purchase.
You may opt out of marketing messages at any time by replying STOP or contacting Glow U Med Spa.
Electronic communications are not encrypted. You accept the risks of electronic communication.
TERM OF CONSENT
This consent remains valid throughout your relationship with Glow U Med Spa unless withdrawn in writing.
